Insurance

Patients are responsible for the services they receive. Our billing department will submit claims to all insurance companies as long as we are provided with valid insurance information. Please bring your insurance information with you to the appointment. Patients who do not have insurance or do not provide us with adequate billing information are responsible for the full payment of their bill. Private pay patients are required to establish payment arrangements prior to receiving services.

Once a claim has been submitted to insurance you will receive a bill for any remaining balance. Patient bills are sent out each month. Co-payments are required on the date of service. It is the patient’s responsibility to know the specifics of their policy. We encourage you to be a proactive participant in your health care and ask that you educate yourself in regards to your individual healthcare coverage plan. If your insurance requires you to have a referral from your primary care office, you are responsible to have that in place prior to your appointment. Call your primary care provider’s referral department for assistance.
